Schedule Change Request Form.docx
- Student and parents must complete Schedule Change Request Form (attached above)
- Completed form may be left at registration site (counselor station) or returned to the front desk.
- If schedule change is approved, student will receive a new schedule on first day of school. If denied, student will follow original schedule issued at registration. Counselors will not contact students to discuss approval or denial of request.
- Due to the nature of the request, if necessary, the counselor will contact the student and/or parent.
